OTA Pending?

I purchased a new phone approx. 2-3 weeks ago.(LG 420G). When I go to the website, it states OTA Pending and no options to refill minutes etc.. are listed. I click on the continue button and it says there is some kind of issue. My service end date is in a few days but can't refill on-line.

This issue seemed to be resolved several days ago, However, it has returned! When I go to the website and log into my account to view my phones, it lists my phone as OTA pending again and nothing further may be done. What is the problem?
