"We cannot process your transaction with the information..."

Hello,

I'm trying to add the straight talk sim card to my Verizon phone. When I attempt to activate it online, I get this message: "We cannot process your transaction with the information provided. Please contact our Customer Care Center at 1-877-430-CELL (2355) for further assistance."
