Cannot add device to straighttalk.com account

Hello,

I've been a customer for about four years now. I just tried to add my phone to my Straight Talk account, but when I put in the phone number / serial number, I get an error message stating "The Serial Number entered is not associated with this account. Please try again." See my attachment.

I have had this particular phone for three years and the number for four years-- how can I get the phone info to show up under my account?

Same problem, website won't let me add a verison sim card to my unlocked iphone 6s. and the apn settings seem like they are for the att sim card i had.
